The Costs of Congestion

Costs are divided into private costs and cost to third parities. The cost to the third party is called an externality. Private costs are costs that are personal to a person or company and so an example of that would be fuelling an airplane with petrol. Externalities occur when a person is doing something, but the full cost of doing the thing is not paid. An example of this is pollution, because no one actually pays for the damage caused. The most efficient outcome to society would be when marginal social cost and marginal social benefit equal each other. The externalities that are caused have to be the main problem. The environment is greatly damaged because of this. The more vehicles driven, the more the pollution caused. 44% of nitrogen oxide emissions in the UK come from cars. Cars also contribute to 24% of carbon dioxide emissions. Both nitrogen oxide and carbon dioxide are big contributions to global warming. Because of global warming, the Earth's average temperature rises which causes sea levels to also rise. Nitrogen oxide also harms the bodies respiratory and immune systems. Carbon monoxide reduces the oxygen carrying capacity of the bloody and could prove to be fatal. The UK road traffic is responsible for 69% of Carbon monoxide emissions. Another externality that is less obvious is noise pollution. This occurs because of vehicles. It causes a lot of disturbance to people. There are very few places left in England where noise pollution doesn't affect anyone in anyway. To reduce car congestion a number of ways have been developed to try and achieve the aim. ‘A new deal for transport' was published which outlined policies to create a better public transport system. The increase use of public transport would mean that congestion would be cut enormously as cars won't be in much usage. The congestion charge in London that was introduced last February was developed to try and cut the congestion problem in the centre of London. This would be effective as people could be put off with the à ¯Ã‚ ¿Ã‚ ½5 charge and would rather use the public transport as it is cheaper. As this scheme is seen to be a long term success, other cities may well introduce them too. A criticism of the congestion in London however is that rich people would be able to pay and the people on lower incomes wouldn't. Although the money gained from the charges would then be used to improve public transport. To carry on using cars and not polluting the environment would be impossible, unless you switch to cars that run on hydrogen. This would be more environmentaly friendly as they emit less pollution than petrol. However the downside of this is that in current times, these cars are very expensive. In the future we may be able to see more of these cars being manufactured and slowly replacing the petrol-run cars.

Reflection on White Privilege

Reflective Analysis #2 Peddy McIntosh highlighted various unearned white privileges in her autobiographical article â€Å"White Privilege, Color and Crime: A Personal Account. † She illustrated the white privilege as an invisible package of unearned assets that one white person could count on cashing in each day. White people have these privileges given to them by the society in which they live in. The same society taught them to be ignorant and unawareness of these privileges. This system of unearned privileges established by white individuals made people of color feel oppressed. In this system being white is a norm and dominant power. Caucasians, who benefit most from the white privilege system in the United States, are more likely to be blinded to the existence of privilege system and take these privileges for granted it. In this reflection analysis, I will elaborate on most common white privileges mentioned by Peddy McIntosh through my personal experiences. As Peddy McIntosh distinguishes white race has unearned advantages over the color race that white people don’t even realize. Caucasians have a special provision, passports, and visas that color people among the world don’t have. For example, individuals from third world don’t get tourist visas to travel around the world and discover new places as the white do. From my personal experience, individuals in India who wants to travel around or settle in a different country don’t have these privileges. Nevertheless, they are unable to achieve their ambition. These individuals don’t have these provisions and privileges because the entire system is established based on white individuals. They’ve granted greater advantage among the same skin color individuals while limiting and lesser advantage towards the color people. Peddy McIntosh made an identical point that United States’ claim to be a democracy in which value is only rewarded to white people. She mentioned white individuals are unaware of these privileges and they are in process of benefitting from the advantages of racism. Another privilege McIntosh classifies is being surrounded by white individuals company. It is easier for a white person to fit in any community because majority of the communities are dominated by their race. For example, if a white person moves in a new house, they are pretty sure greater parts of their neighbors are same race as them. When I moved in the United States many years back, I felt left out and it was difficult for me to fit in because I was a minority. I was mocked by mostly white students about the way I dressed and teased about being different from them. White students don’t encounter these problems when they transfer from one school to another because they figured majority of the students they’ll be surrounded by will be same skin color as them. Another difficulty I had was shopping at a supermarket was finding my cultural food. It won’t be difficult for a white person to fit in a community and find similar food to their culture. A person doesn’t realize these privileges until they image themselves in color individuals’ shoes. In conclusion, Peggy McIntosh pointed in her article, white privileges don’t come from an individual person or flaw of a person. White privileges arise from the flaw of the system placed in a society. Therefore, if we want to change the society and equalize the privilege amongst whites and colors, then change must happen within the system. It is the system that influences individuals to act upon in the society. A great way, we can demise these privileges among white individuals, is through educating the whites and making them aware of these unearned benefits and privileges.

The Strange Case of Dr Jekyll and Mr Hyde Essay Man is no truly one, but truly two. There are two sides to man, a good, and a bad. This book was written by Robert Louis Stevenson in 1885 and is set in the Gothic genre. The gothic genre is associated with darkness, the supernatural, and more importantly, nightlife. The Strange Case of Dr Jekyll and Mr Hyde is based on Stevensons own double life. In his daily life he was a respectable Victorian middle classed man, however by night he lived the life of a debauched bohemian. The Victorian society at that time and the lifestyle was very strict. They believed in morality and formality, and the Victorian children were extremely well behaved. The middle and upper class Victorians would meet at public gatherings, for example church and would expect each other to behave in a very ethical manner. This however was not the case at all times, especially men. They would go out late in the night gambling and drinking. Stevenson portrays this using the life style of a doctor who is ethical, principled and lives a very normal and proportioned life. However, by night his personality and his look grow into a monster like creature. Jekyll is described as tall and handsome throughout the book. He is very well known, and is also very rich indeed. He believes in man having two sides, a good, and a bad. He wants to find out how to separate the two personalities and uses himself to show this. Dr Jekyll, the well respected, good natured man, and Mr Hyde the more primitive and ape like of the two. Mr Hyde is also referred to as simian and hairy. He is considerably smaller than Jekyll and is therefore looked upon as lower in the evolutionary scale. Mr Utterson is the main narrator of this book except for Dr Lanyon and Dr Jekyll in the last chapters. Having more than one narrator in a story is a feature of the gothic genre. Utterson is a middle aged Lawyer and is a person who most of the characters can confide in. He leads a moral life, and has a good profession and therefore is probably the books most respected character. In a way he also has a contradicting personality in the way that he is meant to be a person who is friendly to anyone, however he secretly despises Hyde. By keeping these secrets, Stevenson portrays a double life lurking inside Utterson aswell. Jekyll desires to separate the two personalities of man, however he does not tell anybody due to the fact that people may think that he is trying to play God. Later in the book Jekyll becomes addicted into turning into Hyde, and at one time runs out of the ingredients to make the potion to turn into him. Through this Stevenson illustrates that evil is much more powerful than good. Dr Jekyll keeps his relation with Edward Hyde a secret otherwise people would have looked at him in a different way, however the irony of looking at him as a different man can be perceived by us, the viewers. Near the beginning of this book Hyde tramples a young girl, and the family of that girl ask for his name using blackmail, the alternative however was to pay one hundred pounds in order to keep his identity quiet. This shows how much secrecy meant to the Victorian society. Throughout the book there are references to blackmail between Hyde and Jekyll, (ironically the characters in the book do not know that they are the same person), however the only person who believes that blackmailing has occurred between them is Mr Enfield, who is a cousin of Utterson. Blackmail is slightly engaged with homosexuality and a suggestion to homosexuality is made on frequent occasions during the book. Your master seems to repose a great deal of trust in that young man . Hyde is classed as the Id, as he attacks violently almost in a state of insanity, there is no morality at all in him, he is a total opposite to the nature of Jekyll. He is totally unaware of what he does, and attacks at will. This book is classed in the gothic genre, as it is mainly set in darkness, and it refers to the supernatural and magical things that take place. It presents fear affected on those who live the simplest lives for example, Dr Lanyon who dies purely of fright. There are also multiple narrations, which is a link to the gothic genre. In this book there are three narrators. Other assets linked to this genre, are symbols like the moon, fog, a special door, city streets, good and evil. The door being a very important feature, as it separates two different types of man, Hyde and Utterson. Through this door Stevenson depicts the two sides of man. Frankenstein and Dracula were also written in the gothic genre and similarly link in the same way as Dr Jekyll and Mr Hyde. Frankenstein is about a doctor that creates a monster like man who commits crimes and this therefore links to Mr Hyde who is presented in a similar way in Dr Jekyll and Mr Hyde. Dracula however, is about a man who lives a double life, and plays a perfect host during the day to those whom he may invite to his gothic style mansion in Transylvania. By night however, becomes a creature hankering for blood. The character Dracula is more similarly related to Jekyll as he plays by the aristocratic nature during the day, and by night changes into the unthinkable, for a person with such principle. Dr Victor Frankenstein however is less similarly related to Jekyll as he physically doesnt commit any crimes himself, but its his use of science, which links the two characters together. Stevenson, as a student lived life like many of the men that are presented in this book. He lived in Edinburgh, which had two parts to it, a new town where the middle to upper class people housed themselves and lived moral lives, and an old town where many lower classed people slept on the streets and those who earnt a little more lived in disgusting, diseased filled, houses. Due to educated and well-known parents, Stevenson lived a good life as a child and educated himself at Edinburgh University. It was here where he and a group of friends carried out mischievous things, and then he came to realise that he had two lives, a life where he respected others and was a hard workingman like his father, and also a life where he was free from morality and principles. It was this experience that he had which probably led to this story of a man who is trying to prove that there are really to sides of man, good and evil.

It had an infant classroom, a toddler-two year old classroom, and a preschool classroom. The infants and toddlers were located in the same large room, with dividers that were gates. I focused my attention on the infant and toddlers room. There were three infants and 9 toddlers and 2 -year-olds that were present at the time. They had three full time teachers, one with the infants and the other two were with the toddlers/2-year-olds. During their mealtime was when I first observed the interactions between the infants and the toddlers. Toddlers and infants were allowed to interact with each other during meal time. Infants were in their high chairs or rockers and toddlers were seated near them. I was so amazed to see the interactions between the two age groups. Both groups seem to really get along with each other. I observed a two year old girl feed an infant around the age of 6 or 8 months old. They were both smiling and enjoying each others company, of course with adult supervision. Another toddler was allowed to feed another infant a bottle, and the toddler had this huge grin on his face as to be so proud when the infant allowed him to feed him. During their play time I observed some toddlers were more interested in the toys the infant had then toys they had. One of the toddlers would frequently go to one of the infant rockers trying to climb in instead of playing with toys that were there for her. I also observed the toddlers playing with baby dolls. One thing that caught my eye was how they were handling the doll with such care. One of the toddler girls sat the doll in her lap and grabbed a book and pretended as though she was reading to the doll, while another toddler was rocking and singing to her doll. In another section of the room I observed three toddler boys playing. One of the boys had just turned three years old and the other two boys were 1 year old. The older toddler wanted to lead and control the play. He would make suggestions on what to play and the others would follow along. Also, when the younger boy toddler tried to leave and go to another area, the older toddler guided him with his own type of language ba ck to the group. Analysis of play and interactions Infants respond positive to toddlers when interacting with one another. Field (1990) found that infants smile at, look at, and reach more often to peers when seated near each other than they do when seated in front of a mirror. The National Institute of Child Health and Human Development (NICHD, 2005) indicated that children in early care experiences become more positive and less negative toward peers in their play between 24 and 36 months. In observing the environment alone, it made it conducive for the positive interactions between infants and toddlers. It was possible for them to see each other at all times even though their classes were divided. The gates made it possible for them to be separated and at the same time, allowed them to have visual contact at all times. For example, during meal time, infants were seated at the same table with the toddlers/2year olds at eye level and the 2-year olds were definitely showing strong interest in the infants and their well being. This shows that when they have the opportunity to closely interact with one another, they have a trusting relationship. Description of use of cultural objects and language use There were dolls and other materials in the toddler and 2-year-old classroom. The children imitated the interactions they observed between infants and adults in the classroom. For example, how the toddlers were handling their dolls was much the same as adults used in caring for the infants. Some things toddlers were doing when playing with the dolls were feeding, comforting, singing, rocking and reading to their doll. I also observed a male toddler that had just turned 3 years old interacting with the younger toddlers that were 1-2 years old. I called this 3 year old toddler the alpha toddler because he led the group with ideas of play. The younger toddlers responded with acceptance and trust. I think this gave the 3 year old a since of responsibility of being a leader as well as looking out for them. For example, when one of the 1-2 year old toddler decided to wonder off to a section I assume he wasnt suppose to be in, the alpha toddler ran after him telling him, no, no, and the 1-2 year old returned with the others and continued his play. The last thing I observed before I left the facility was an infant and a toddler playing and interacting with each other. They were both playing with some sort of plush toys really enjoying and exploring them. They were making eye contact with one another, returning their smiles, making gestures, reaching to communicate with each other. When the infant dropped his toy he was playing with, the toddler picked it up and handed it to him and the infant nicely received it from him with a smile and they continued to play and explore with their toys. Piaget, Erikson and Vygotsky all agree that the child uses play for self teaching. The child plays through situations very much like an adult thinks through a situation. Also, fantasy play is a manifestation of symbolic representation the child represents objects and ideas through play situations. Vygotsky believed that play is a means of deferring immediate gratification-instead of tantrums or swallowing the need; the child fulfills needs in fantasy play. He also believed that children learn to live within self imposed rules during fantasy play; play allows the child to practice self regulation. Play, for Vygotsky, was vehicle for a child behaving more maturely than a other times. In play it is as though he were a head taller than himself. In 1920, Sigmund Freud posed a psychoanalytic play theory that was defined in his book, Beyond the Pleasure Principle. Lev Vygotsky suggested that children will use play as a means to grow socially. In play, they encounter others and learn how to work together using language and role-play. Vygotsky is most noted for introducing the ZPD, or zone of proximal development. This suggest that while children need their peers or playmates to grow, they need adult interaction as they master each social skill and are ready to be introduced to new learning for growth. Reflection I really enjoyed the time I spent at the daycare facility. The classroom spaces for their infants and toddlers provided opportunities for the younger and older children to interact with each other. I think through their interactions and play, the infants will learn certain behaviors from the older children, such as feeding themselves, walking, running, etc. Things that will possible make transitioning stages from infant to toddler easier as well as making the transition from toddler to preschool easier. Allowing the toddlers to assist with things such as feeding the infants and playing with them, and in their mind helping them, allows the toddlers to have a since of maturity and independence and not scared and dependent on others do everything for them. Genealogical Experiment of Fish Types David Hess Abstract: To introduce us to proteins, which truly make one organism different from another in terms of phenotype, our instructor challenged us to study the physical and proteomic traits of salmon, catfish, turbot, halibut, and yellow-fin tuna and estimate how each fish is related on the evolutionary tree. To do so, our lab group first accessed online research websites to compare the phenotypes of the different Ichthyoids. We specifically researched sizes, swim types, biological features, habitat preferences, and taxonomic names that derive from the evolutionary tree. After this, we then obtained samples of the muscle tissue in each fish, which were provided by the instructor, and then extracted the proteins from each sample. By treating the samples with sodium dodecyl sulfate and applying heat. We were able to denature the[D1] tertiary and quaternary structures of the proteins, which left the proteins long, stringy, and negatively charged. Next, we were able to separate the proteins by length a la gel electrophoresis, and compare the different proteins in the fish as we observed the different bars that appeared on the gel. After comparing both the physical traits and the proteins in each fish, we were to predict which species preceded the next according to evolution. Purpose: The purpose of this lab is to study the physical attributes and proteomics of different species of fish to determine the potential genealogical tree connecting these species Instructor/Background: Proteins often bind together, forming polypeptide chains. Discussion: When reviewing the data once more, we noticed some discrepancies in our cladogram compared to the evolutionary tree. For example, our Yellow-fin Tuna found its way onto the beginning of the tree, when it should have landed near the end according to the evolution tree in our packet. Otherwise, we believe this experiment [D6]was a success, as we learned about how proteins can be used to supplement genetics and give us another tool in understanding our history. This could possibly be result of contamination in the fish muscle samples, due to touching the muscles with the same pair of gloves when transferring them into the tunes for protein extraction. If we were ever to do this experiment again, we would be sure to use tweezers of another similar tool to handle the muscles. Although many of her publications were magazine submissions (The New Yorker), Bishop released different collections of her poems. Questions of Travel (1965) focused on many of the settings she saw and felt while living in Brazil. Brazil (1967) was a travel book of poems about Brazil's surroundings. An Anthology of 20th Century Brazilian Poetry (1972) is exactly what it labels, Brazilian poetry. Highschool Stereotypes Essay

When you first enter high school, you can face many different difficulties. (exams, new friends, drama etc. ) But one of the most stressful difficulties of all are stereotypes. ‘What is a stereotype? Where do I fit in? Do I Have to fit in? ’ are just some of the things you may be worrying about. There are three main stereotypes that I have come to recognize that are in almost every single school. The preps, the nerds, and the emos. I will help you classify which is which and understand the difference so that you can determine whether you want to fit in with these groups, or go your own way. The first main stereotype is the preps. They will usually be well talked about and you will hear their names often. They dress nice in expensive clothing, are usually wealthy and are sometimes in sports like football or cheerleading. This stereotype is usually filled with good-looking people and these types of kids are known to be meaner than average. There are advantages and disadvantages to being one of the preps. Some good things about being one is that you are well known, people envy you and want to be your friend. You usually have fun because you are invited to parties and everyone wants to be around you. A few disadvantages are that you’re expected to dress nicely at all times, you are also supposed to act accordingly even if that means being rude to someone else. The second stereotype is the nerds. These kids are the exceptionally smart ones. These are the people in your class that get straight A’s. They enjoy doing homework and extra credit and are usually known to be socially awkward. They often wear glasses or braces or breathe heavily and often get bullied. Just like every other stereotype I am talking about today, being a nerd also has it’s own ups and downs. Some advantages are that you get excellent grades, which makes teachers impressed, and parents proud. Also, since you’re academically higher you are more likely to get into a good college and get an excellent education, which will probably lead to a high paying job in the future. Some downsides are that you won’t be invited anywhere fun, probably won’t have many friends and might get picked on a lot by classmates or bullies. The third stereotype is the emos. This is probably the easiest stereotype to spot out. They dress in all black, very dark makeup, dyed black hair and are usually depressed and don’t speak to anyone outside their circle. They frown upon the normal members of society and think they are ‘different’. They are usually sad and their conversations revolve around how they want to die and how terrible life is. These kids despise the preppy kids and hate anything happy or cheery. They usually spend a lot of time skipping class together, and chatting online to each other. The upside to joining this stereotype is that they have an odd sense of community. They all stick together no matter what and listen to each other’s problems. Some of the downsides are that you are looked down upon by most people in society, and most people do not want to approach you or get to know you. It will be hard for you to get a decent job, and people will make bad assumptions about you. Well, now you know the 3 main high school stereotypes. The preps, the nerds, and the emos. You can be mean and popular with the preps, be geeky but have good grades with the nerds, or be depressed but have a sense of community with the emos. You can weigh out the positives and negatives for each and decide if one of these groups is right for you or you can decide to just be yourself and go your own path.

The world is governed more by appearances than by realities. Throughout society people are judged based on their appearance. This is unfortunate because appearance can be deceiving. In William Shakespeare’s play Macbeth appearances are not what they seem. Some people may change because they think that their true self does not fit in society while others pretend to be something they are not. In the theme of appearance versus reality Macbeth and Lady Macbeth have an image but as the time passes by their true personalities began to show. Macbeth portrays himself differently from his true inner self. He pretends to be a strong and intelligent person but from inside he is weak and indecisive. When Macbeth first confronts the witches’ prophecies he says, â€Å"Come what may, time and the hour runs through the roughest day† (1. 3. 152-153). Macbeth is leaving his future to ‘chance’. He wants to appear noble and strong but in the end he refutes his statement by killing Duncan and Banquo to get to the throne. Also, when Lady Macbeth explains the plan to Macbeth he asks â€Å"If we should fail? † (1. 7. 59). He is still afraid of the consequences of his actions. His actions look pungent but he does not have any reason to kill Duncan. Macbeth has a debate within himself to murder King Duncan or to spare his life. Macbeth hesitation to kill Duncan proves in realty he is a weak man. Lady Macbeth represents herself as a beautiful and powerful woman in society but she is as weak as her husband Macbeth. To convince Macbeth to follow the plan she says â€Å"I have given suck, and know how tender ’tis to love the babe that milks me. I would, while it was smiling in my face, have plucked my nipple from his boneless gums and dashed the brains out, had I so sworn† (1. 7. 54-58). Macbeth asks for her help, but she responds by being harsh to him to keep up her appearances. Even when Macbeth sees Banquo’s ghost, instead of helping him, she tells him to go to bed and stop imagining weird things. Lady Macbeth appears heartless toward others. However, one night a gentlewomen and a doctor see her sleep waking and mumbling â€Å"Out, damned spot! Out, I say! † (5. 1. 25). She appears to be wide awake but she is unconsciously revealing her true self. Her guilty conscious leads to her downfall through suicide. In this scene her appearance crumbles. She seemed powerful and unsympathetic, but in reality her guilt of Duncan’s murder driver her crazy proving she has become weak. In Macbeth the theme of appearance versus reality is shown through the characters of Macbeth and Lady Macbeth. They both revealed a fictitious picture of their personality even though on the inside they are different people. Macbeth’s indecisive personality leads to his destruction. The iron lady, Lady Macbeth appears to be a tough woman, who feels no guilt. But in the ending she shatters to pieces because of her guilt-ridden conscious. No matter what one appears to be reality will defeat appearance. People can put disguise on reality for time being but in the end truth reveals itself.

Ventilator Associated Pneumonia in the Icu

Ventilator associated pneumonia (VAP) is a nosocomial infection occurring in hospitalized patients who are mechanically ventilated. These infections are common in ICU settings, difficult to diagnose early, and unfortunately have a high rate of mortality and morbidity. VAP accounts for almost half of infections in ICU settings, up to 28% of mechanically ventilated patients will develop VAP and of these patients the mortality rate is between 20% and 70% (Craven & Steger, 1998). A patient that develops VAP while mechanically ventilated adds days to his recovery as well as thousands of dollars to the care costs. Numerous studies have been conducted across the county in an effort to understand VAP, however very few of those studies focus on the nursing interventions that can prevent this deadly and costly nosocomial infection. Many of these studies focus on the â€Å"bundling† of certain interventions, so the question is does the implementation of a VAP bundle compared with the use of non-bundled interventions decrease the incidence of VAP in ventilated patients. The research that was found in nursing journals along with a related study from a medical journal follows. Cason, Tyner, Saunders and Broome (2007) conducted a study of 1200 critical care nurses and the results demonstrate the variability in the recommended and reported care of the ventilated patient. Their study aims to identify the areas of needed improvement to comply with the CDC recommendations for prevention of VAP. The study consisted of a questionnaire distributed to nurses who attended the 2005 American Association of Critical Care Nurses National Teaching Institute, with the findings demonstrating a need for more education and research in the area of preventing ventilator associated pneumonia. Ferrer and Artigas (2001) also noted the lack of compliance in even the most basic of preventative measures. The study focuses on non-antibiotic preventative strategies for VAP; they suggest the use of antimicrobial hand soap, clorahexidine oral rinses, stress ulcer prophylaxis, prevention of gastric over distension, providing adequate nutritional support as well as frequent position changes. The research also suggests that endotracheal tubes with an extra lumen designed to continuously suction secretions pooled above the endotracheal tube cuff would lower the incidence of VAP by reventing these secretions from being aspirated into the lower airway. The authors also suggest more research be done to further limit the number of patients who develop VAP. Siempos, Vardakas and Falagas (2008) found that after meta-analysis of nine published randomized controlled trials that a closed tracheal suction system has no benefit in reducing the incidence of VAP compared with an open tracheal suct ion system. The preliminary data suggests that a closed circuit would reduce the incidence of VAP, however the data and trials that were reviewed showed that there was no decrease in the rate of infection. Due to the fact that a closed system can be used more than once, and only needs to be changed every 24 hours, it does tend to be more cost effective. Also of note, in two separate trials, a closed system was found to increase colonization of both the respiratory tract and the ventilator tubing. Obviously more research is needed to determine the best intervention when endotracheal suctioning is necessary. Ventilator associated pneumonia is both common and unfamiliar to nurses in the critical care setting, according to Labeau, Vandijck, Claes, Van Acken & Blot (2007). They note that while nurses deal with VAP frequently their knowledge of the infection and preventative measures may be a reason that VAP is still so prevalent in the intubated patient. The research points to the idea that because many nurses are not responsible for the ventilator circuit; they rely on the respiratory therapist to manage the ventilator, they may be less informed than if they had more control and training in the interventions necessary to prevent VAP. The research suggests more training and education for nurses who work with ventilated patients. Research done at the University of Toledo College of Medicine has shown a decrease in the incidence of VAP in its ten bed surgical ICU by implementing a â€Å"FASTHUG† protocol. Papadimos, et al, (2008) explained the interventions that the college used as a tool to educate the critical care team. â€Å"FASTHUG† stands for daily evaluation of feeding, analgesia, sedation, thromboembolic prevention, head of bed elevation, ulcer prophylaxis, and glucose control in critically ill intubated patients. The â€Å"FASTHUG† protocol was emphasized at morning and afternoon rounds and after a 2 year esearch period the incidence of VAP declined to 7. 3 VAPs/1000 ventilator days down from a historical rate of 19. 3 VAPs/1000 ventilator days. Of note, in 2007 the surgical ICU that implemented this program actually had no incidence of VAP from January to May. The research suggests that the use of bundled care processes for ventilated patients may reduce the rate of VAP. The nurs e’s knowledge of the use of the ventilator bundle is crucial to the success of the protocol according to research done at the University of Texas. Education sessions were held with pre and posttests administered as well as observation to evaluate the nurse’s understanding of the bundles. The VAP bundle focused on the elevation of the head of the bed, continuous removal of subglottic secretions, change of the ventilator circuit no more often than every 48 hours, and washing of hands before and after contact with each patient. The research done by Tolentino-DelosReyes, Ruppert and Shiao (2007) suggests that a lack of understanding and knowledge of VAP leads to a higher rates of infection. Observation of the nurses in the study revealed that after the education sessions nurses demonstrated an increase in compliance with the established standards of care. Given the high mortality and morbidity of ventilator-associated pneumonia, compliance in the critical care unit is crucial to reducing the rate of VAP. I suppose I’d call it book-origami, but I’m not sure if there’s a proper title for this sort of thing. My purpose tonight is to teach you how to make one of these. The way I have been making these has been without the use of any glues or starches, and the hardcover frame seems to help the book hold its shape naturally. B. When choosing your book you will also need to consider the number of pages that it has. 2. This was actually my first book, and it has about two hundred fifty pages in it. My second book actually has over 550 pages in it. The choice you make depends on your taste, but personally I think the thicker book has a much fuller and professional look. C. You should also consider the size of the cover of your book. 1. I believe that a someone smaller book makes for the best decoration because it doesn’t take up much room. Transition: Now that you have chosen a book to work with, you are ready to divide your book in sections. A. In my first book I didn’t take a whole lot of time, and just approximately divided the book into 3 sections. 1. You might not be able to tell at first glance but one side actually has around 15 pages more using the first folding style. B. If you want to go for a truly symmetrical look, I would suggest counting out each page individually. It will take more time this way, but you will probably be happier with the end result. 1. After you have decided how much you want in each section, I would recommend marking each sections beginning and end with a bookmark. Transition : Now that you have sectioned off your book, you’re ready to start folding. (please excuse my lack of technical terms, but this is really just something I’ve been experimenting with and was not actually taught anything about origami. A. I like to start off each book with several simple corner folds. 1. After each page, you should alternate between folding the top corner down, and folding the bottom corner down. 2. The reason I start the book this way because it is a really easy technique, and it also seems to help the book expand quite a bit. B. Another fold I use quite a bit I would call an airplane fold. I call it this because it is how I remember starting off paper airplanes, again this isn’t a technical lesson. 1. This technique is done simple by folding the top and bottom edges into the middle of the page. 2. I tend to use this technique for the middle section because again, it is very easy to do, and it helps the book to stand up. C. What I have used for the outer-middle section here is actually just a variation of the airplane fold. 1. After folding both edges in, you then take the middle point of the page and fold it to either the upper or lower portion, again alternating with each following page t o get this accordion look.
